Sometimes you got to drink irradiated water or eat a person else you're character will die. Survive and live to loot another day. I remember in Fallout 3 being trapped in a firefight with supermutants in a hotel drinking water out a dirty toilet bowl to keep my character alive. And if you pick up the cannibal perk there's always food around.

One clarification about the Mr. Handy (the robot): it is not sentient. Everything it does is because either due to being programmed that way or a malfunction. They were used pre-war as doctors or handymen. Its chatter are programmed responses to look like a doctor's bedside manner, and it probably healed her finger not because it reasoned it had to keep her alive to harvest her organs, but because its what it was programmed to do. It was probably later reprogrammed to do the harvest. That's why there's such a mistmatch between the way it acts and the way it speaks.
