the egg is a metaphor for periods, but later in the episode it's also a metaphor for unplanned pregnancy. In the conversation between Touga and Nanami, he thinks she's talking about being pregnant, and is telling her they've gotten along so well because she's not the type of girl who would put herself in a situation where that's a possibility, and saying that his love for her is conditional on her remaining that type of girl (i.e celibate, a good girl, a "princess", on the madonna side of the madonna/whore dichotomy). It really hammers home for Nanami how conditional his love for her is, which is also what makes it so meaningful that she cared about the egg enough to go back for it, even though in her mind doing so is risking her brother's affection for her. We see that Nanami really does have a heart buried under her mountain of teenage insecurities (that Touga feeds into and makes worse).

I do wonder if in the sane version Nanami thinks she might be pregnant because she doesn't know how periods and pregnancies work. Possibly because she isn't paying attention in health class since Tsuwabuki does all of her work for her or alternatively this was the first health class she had on the subject but was too distracted to pay attention. Her acting like she's expecting a human to be born is what leads to the conversation about having "a bun in the oven" so to speak. Though the egg as a pregnancy metaphor now makes the rest of the show rather dark, with Nanami signing up for an abortion, but pulling out at the last minute then miscarrying and not being able to share that pain with her family. However, if the egg remains more of a period metaphor I am unsure what the leaving it in the woods section would be read as. Though it breaking could mean that her period ended. This episode is almost a PSA as to why sex ed early is so important. Nanami being embarrassed goes to other kids first in a panic and gets incorrect information partially because she's withholding out of fear of embarrassment. When she's told that it is normal, she then runs away from getting more information or support from friends because she's afraid of mockery for her ignorance about life and her own body and also potentially still having something wrong with her (too late or too early). Trying to speak about it only in code, leads to further misunderstandings. Revelations about what the people around you consider acceptable or not from your reproductive cycle usually pop up around this time too. There's always one person who piles on more shame and makes it known that their support is conditional based on what you choose to do with your body.
